This form should be completed to request assistance to attend the Special General Meeting held on 17 July 2024 at the AMYAC Office in Coober Pedy.

To make a claim the form must be submitted
Before 4pm WEDNESDAY 3rd JULY 2024.

For Members requesting assistance for food/fuel, electronic gift cards will be issued where possible. Members must have an email address to accept gift cards. If you require assistance with this, please contact the Executive Office.

Accommodation – AM-YAC members are to book and pay for their own accommodation. AMYAC members that attend the meeting can arrange reimbursement for a maximum of two nights’ accommodation by providing a receipt to the Executive Office.

The name on the receipt needs to be in the name of the AMYAC member or nominated person. Reimbursement will be processed within 3-4 working days of receiving receipt and bank account details.

Please note:

• Once applications are approved, no changes can be made.
• No late applications will be processed. If an application is received after the cut-off date, it will not be processed.
• Members who receive assistance to attend the SGM and not sign in without a valid reason will incur a debt and will be placed on the Poor Conduct Register. This debt will be recovered from future AGM assistance applications.
