AGM Assistance Form
This form should be completed to request assistance to attend the Annual General Meeting held on 13th – 15th November 2024 at the AMYAC Office in Coober Pedy.
To make a claim the form must be submitted
Before 4pm WEDNESDAY 23rd OCTOBER 2024
Fuel – For Members requesting assistance with fuel electronic gift cards will be issued where possible. Members must have an email address to accept gift cards. Members requesting assistance with fuel must provide registration details of car and valid driver’s licence.
Food – A Gift card (no tobacco, no alcohol) of $200 to purchase food during the AGM will be available for collection from the AMYAC corporation the day before the meeting or on sign in only.
Accommodation – AM-YAC members are to book and pay for their own accommodation. Reimbursement for a max. of four nights’ accommodation at a maximum of $200/night can be requested by providing a receipt to the Executive Office.
The name on the receipt needs to match with the AMYAC member and the bank details to be reimbursed. Reimbursement will be processed within 3-4 working days of receiving receipt and bank account details.
Please note:
- Once applications are approved, no changes can be made.
- No late applications will be processed. If an application is received after the cut-off date, it will be declined.
Members who receive assistance to attend the SGM and not sign in without a valid reason will incur a debt and will be placed on the Poor Conduct Register. This debt will be recovered from future AGM assistance applications.
