Health and Medical – Form
Up to $5,000 per year per Member may be available for the following when not already covered by Medicare:
- Doctor’s appointments, x-rays, scans, medical tests and medical treatment prescribed by a medical practitioner.
- Dental treatment.
- Glasses and optical treatment.
- Physiotherapy and podiatry.
- Medication prescribed or recommended by a medical practitioner.
- Preventative health measures recommended by a medical practitioner, e.g. counselling and/or quit smoking programs.
- Reasonable travel and accommodation expenses for appointments more than 75km away from place of residence.
- Dialysis treatment related travel (food, fuel, bus ticket) up to $150 each week travel required.
- Airline tickets in emergency situations only (Members are responsible for their travel arrangements and may incur a debt for missed flights).
- Ngangkari treatment (refer separate application form).
Support provided to AM-YAC children is taken out of their parents’ annual entitlement. (or grandparents’ annual entitlement if applicable).
It is recommended an application to the PATS Program be made prior to an application to the Trust. Members who submit a successful PATS claim will receive funds back into their medical bucket.

SERVICE STANDARDS
If your application is consistent with trust policies and budget you can expect approval within three business days.
