SGM Assistance Form
This form should be completed to request assistance to attend the Special General Meeting held on 17 July 2024 at the AMYAC Office in Coober Pedy.
To make a claim the form must be submitted
Before 4pm WEDNESDAY 3rd JULY 2024.
For Members requesting assistance for food/fuel, electronic gift cards will be issued where possible. Members must have an email address to accept gift cards. If you require assistance with this, please contact the Executive Office.
Accommodation – AM-YAC members are to book and pay for their own accommodation. AMYAC members that attend the meeting can arrange reimbursement for a maximum of two nights’ accommodation by providing a receipt to the Executive Office.
The name on the receipt needs to be in the name of the AMYAC member or nominated person. Reimbursement will be processed within 3-4 working days of receiving receipt and bank account details.
Please note:
- Once applications are approved, no changes can be made.
- No late applications will be processed. If an application is received after the cut-off date, it will not be processed.
- Members who receive assistance to attend the SGM and not sign in without a valid reason will incur a debt and will be placed on the Poor Conduct Register. This debt will be recovered from future AGM assistance applications.
