Carer’s – Application Form
Up to $1,000 per year to support AM-YAC Members and their immediate family where a carer is required to support them with a medical condition (e.g. to attend appointments or receive medical treatment).
Medical conditions might include:
- A disability
- Are under 18 years of age
- A serious medical condition
- Aged or frail
- Travelling for medical care.
The carer must be nominated by the AM-YAC Members’ family.
* A support letter from a doctor or health professional is required.
To apply, submit the form.
You will receive a confirmation email when your application has been received.
SERVICE STANDARDS
If your application is consistent with trust policies and budget you can expect approval within three business days.
