PATS – Application Form

The Patient Assistance Transport Scheme (PATS) provide assistance to people who live in rural South Australia who need to travel more than 100 kilometres each way to see a medical specialist that is not available locally.

To apply for PATS there are three documents to complete.

1: Letter signed by the referring doctor in your home town.
2: Letter signed by the specialist on the day of your appointment.
3: Image of your concession card.
4: This application signed by you as the AMYAC Member.

We encourage all members attending non-emergency medical appointments to submit PATS forms. The amount refunded under the PATS scheme is returned to the member’s Trust entitlement balance.

Forms, 1, 2, and 3 and the concession card image are required by SA Health to determine eligibility to PATS funding.
